Se hereda con car\u00e1cter autos\u00f3mico dominante, teniendo una penetrancia del 85%. El concepto de penetrancia se refiere a la proporci\u00f3n con la que se presenta un gen dominante en la familia, es decir que en este caso, un miembro de la familia que hereda el gen con una variante de riesgo tiene un 85% de probabilidades de manifestar el c\u00e1ncer, e independientemente, la probabilidad de heredarlo a su descendencia es de 100%.<\/p>\n<p>&nbsp;<\/p>\n<p>El c\u00e1ncer hereditario de mama y ovario pertenece al grupo de los s\u00edndromes de predisposici\u00f3n a c\u00e1ncer, ocasionados por mutaciones en genes encargados de la integridad gen\u00f3mica, debidas a mutaciones en las v\u00edas de reparaci\u00f3n del ADN, produciendo inestabilidad gen\u00f3mica.<\/p>\n<p>Al menos el 25% de las pacientes con c\u00e1ncer hereditario de mama y ovario tienen mutaciones germinales en BRCA1 y BRCA2. En la base de datos del Breast Cancer Information Core de los Institutos Nacionales de Salud de los Estados Unidos de Norteam\u00e9rica, se han identificado m\u00e1s de mil variantes patog\u00e9nicas en estos genes, lo que indica la necesidad de hacer estudios completos de secuenciaci\u00f3n en lugar de hacer pruebas de sitios espec\u00edficos, basados en las frecuencias que presentan ciertas poblaciones.<\/p>\n<p>Se sabe que el mecanismo de las variantes registradas act\u00faa cambiando en dos formas: 1\/ cambiando el marco de lectura del ADN, a trav\u00e9s de deleciones o inserciones de nucle\u00f3tidos, lo que produce un corrimiento del marco, generando prote\u00ednas anormales, o bien 2\/ produciendo mutaciones sin sentido, en la cual una tripleta alterada codifica para un codon de paro, lo cual conduce a la formaci\u00f3n de prote\u00ednas truncadas.<\/p>\n<p>Dado que los genes BRCA son tumor-supresor, su funci\u00f3n es reparar el ADN da\u00f1ado, haci\u00e9ndolo a trav\u00e9s del mecanismo de recombinaci\u00f3n hom\u00f3loga, toda vez que se trata de rupturas de doble cadena del ADN, se acompa\u00f1a de complejos mecanismos moleculares, en los que participan PCNA, Rad50\/51 y BRCA2, formando un conjunto de prote\u00ednas cooperadoras que orquestan el proceso de reparaci\u00f3n de rupturas de doble cadena.<\/p>\n<p>Como en muchos otros ejemplos, la variabilidad gen\u00e9tica en los diferentes tipos de c\u00e1nceres hereditarios, el de mama y ovario, no solo tiene una causa, se ha comprobado que tambi\u00e9n se asocia con mutaciones germinales en los genes PTEN y en TP53.
